Most practices choose between two options: an in-house biller or a billing company that takes over the whole revenue cycle. This guide covers a third one, a dedicated remote billing specialist who works inside your own systems, and when each option makes sense.
What does a medical biller do?
Submits claims to payers, verifies eligibility and benefits, follows up on unpaid and denied claims, posts payments, sends patient statements and answers billing questions. In many practices the same person also handles prior authorizations.
The work lives in your practice management system or EHR, your clearinghouse and the payer portals. A dedicated biller works in those same tools every day, with the access your practice grants.
Should I outsource billing to a company or hire a dedicated biller?
A billing company takes over the process end to end, usually with its own team and its own reporting, and is often priced as a share of what it collects. It suits practices that want billing off their plate entirely.
A dedicated biller suits practices that want to keep the process, the system and the visibility in house, but cannot find or afford the person locally. You direct the work, and the claims and notes stay in your system.
When is a dedicated remote biller not the right fit?
When nobody in the practice can own billing. A dedicated biller needs someone to answer coding questions, approve write offs and decide on appeals. Without that owner, a billing company that runs the whole cycle is the better choice.
It is also the wrong hire when the real problem is upstream: wrong registration data, missing documentation or coding that the providers have not settled. A biller can surface those problems, but cannot fix them alone.

Red flags in medical biller candidates
Resubmitting without reading. Given your sample denial, a candidate who goes straight to sending the claim again, without reading the reason or choosing between a corrected claim and an appeal, will send the same denial back to the same payer.
Tickets closed, not dollars recovered. Ask how they would work the aging report on the first day. If the answer is to start at the top and go down the list, rather than oldest and largest balances first, your old receivables will keep aging.
Accounts without notes. A candidate who does not mention leaving a note on every account they touch leaves the next person, and you, guessing where each claim stands.
Denials as one offs. Ask whether they have seen the same denial keep coming back from one payer and what they did about it. Someone who only ever fixed the claim in front of them will never surface the problem upstream.
Improvising with patient data. If they describe using personal tools, accounts or shortcuts to get work done faster, they will do the same outside the access and policies your practice sets.
How long before a new medical biller is effective?
Expect the first two weeks to go on access, your payer mix, your fee schedules and how your practice handles write offs and appeals. By the end of the first month they should be working submissions and denials on their own.
